Our Initiatives
Lead Home Care Agency
The Lead Home Care Agency (LHA) model is a new model of care that provides a more seamless experience for funders, partners, patients, caregivers and health care providers to improve overall patient outcomes and experience while also being more cost-effective. The LHA model provides enhanced and more integrated one-stop supports throughout a patient’s care journey, including the transition from hospital to home.
Transitional Care Programs
Acting as a pressure-relief mechanism, VHA works with hospitals to safely transition individuals receiving care in acute care beds, inpatient hallway beds or those waiting to be admitted to the ED who no longer require acute care, providing care for them in transitional spaces. The Transitional Care Program (TCP) includes Transitional Care Units which serves as a transitional space for clients to receive the level of care they require, which includes personal support, nursing, physical therapy, occupational therapy and additional supports as needed, in a community setting prior to discharge to their destination: home, long-term care, assisted living or hospice. The TCP also includes high intensity supports in the home to support clients and their families/caregivers in re-introducing the client into their home.
Healthcare Response Unit
In response to the changing needs of Ontario’s health care system, in 2021, VHA created THRU, our Tactical HealthCare Response Unit. THRU is a strategic team of experienced nurses, pharmacists, pharmacy assistants and non-clinical staff who can urgently respond to community health needs and challenges anywhere in the Greater Toronto Area by either in-person or virtual means. The THRU team is configurable to the needs and as such can include an integrated team pulling from a full basket of health care and community providers.
Research and Innovation
VHA is dedicated to supporting research and innovation that advances human welfare, creates new knowledge, develops our understanding and informs best practices in home and community care. Our research focuses on key areas of home and community care that create possibilities for more independence, support clients to live at home, promote safe and rewarding working conditions for home care providers, and build toward fully integrated care across sectors. We strive to break new ground in research and development of integrated solutions that reflect the voices of our clients, caregivers and providers.

We have experience working with many health system partners/funders across a variety of settings (in home, community and inpatients), and can tailor service delivery to specified requirements. VHA is committed to delivering high quality care and making a positive impact with all our partners to achieve desired outcomes.
Partnering with Primary Care
VHA actively coordinates and collaborates with primary care providers and family health teams, helping improve the quality of care provided to individuals in our care. VHA partners with primary care providers to better understand the needs of clients, responding with individualized specific services that create the best impact on the health of our clients and their communities. Our collaboration with primary care providers has led to improved client care outcomes through improved client flow through programs, integration of remote patient monitoring, and improved continuity of care.
Ontario Health Team Participation
VHA is a member in Ontario Health Teams in the regions where we provide service. VHA actively contributes in the areas of integrated care model development and service delivery, governance, operations, digital services, privacy, data and research and communications.
