Home and Community Care Support Services is now called Ontario Health atHome

On June 28, 2024, as part of the Ontario government’s plan to modernize home care, the 14 local Home and Community Care Support Services (HCCSS) organizations were combined into one new organization called Ontario Health atHome. As a result of this change, Home and Community Care Support Services is now called Ontario Health atHome.

Ontario Health atHome will continue to be responsible for coordinating home care across the province. Clients and families will continue to have access to the services they need through the very same people and providers. All contact people and their phone numbers remain the same. Any email communication with Ontario Health atHome contacts should use email addresses in this format going forward: [firstname].[lastname]@ontariohealthathome.ca.

This change is related to the transition under way in Ontario to make care more integrated through Ontario Health Teams. For the time being, there is no change to the way homecare services are provided.
